Waiver/Release

I hereby waive and release, indemnify, hold harmless and forever discharge Facets of Novahs Hair Artistry, hereafter referred to as “FONHA” and its assigns, employees, owners, agents, and affiliates, of and from any and all claims, damages, liabilities, and lawsuits of any kind of nature, whether known or unknown, that arises in relation to the services being provided to me by FONHA provided that this waiver of liability does not apply to any acts of gross negligence, or intentional, willful, or wanton misconduct. Said services may include, but are not limited to, hair adhesive, braids, chemical processing, texture changes, hair extension installs, wig installs, as well as eyelash application.

I assume any risk, and take full responsibility and waive any claims of personal injury or damage to personal property associated with such services provided by FONHA including but not limited to temporary or permanent damage to my hair or undesirable results from the said services being provided to me. This Waiver and Release voids any prior written or oral agreements concerning the subject matter of this Waiver and Release. The provision of this Waver and Release will remain in full effect even after the termination of services.

By paying the deposit, the client acknowledges and agrees with the terms and conditions above.
